Position: 21st Century Case Manager
Reports to: 21st Century Learning Center Grant Supervisor
(related to Learning Center Activities) and CISNT Campus Manager (related to CISNT policies and procedures)

PURPOSE:
Serve as the Communities In Schools North Texas (CISNT) Case Manager for one of the five Texas 21st Century School Is Out; Learning Is In Learning Centers. Coordinate instruction, enrichment, mentoring, and parental involvement activities for after-school and summer programs. Facilitate parental involvement with at risk students and family members for the purpose of decreasing dropout rates, and increasing student academic performance, behavior, attendance, grade level promotion and graduation from high school rates. Collaborate with Instructional Specialist and Mentor Partnership Coordinator to provide case management for students and families served as part of the after-school and summer Learning Center program. Document services provided.

QUALIFICATIONS:
Minimum Education/Certification:

  • Bachelors Degree in a human service field (Social Work, Education, Sociology, Psychology, Counseling, Human Relations, or related field
  • Minimum of one-year experience working with children and adolescents preferred.
    Bilingual Preferred

Special Knowledge/Skills:

  • Experience working with economically disadvantaged, at risk and/or English as Second Language Learners
  • Creativity and originality to meet assist students in developing higher level reasoning, decision making and problem solving skills
  • High levels of initiative, flexibility and organizational skills; ability to perform duties with limited supervision
  • Computer literate required

MAJOR RESPONSIBILITIES AND DUTIES:Campus-based School Is Out; Learning Is In 21st Century Learning Center Case Manager.

Program Management

  • Coordinate with Grant Supervisor and Mentor/Partnership Coordinator to provide after school academic and enrichment programming, mentoring programming, parent programming, and summer programming.
  • Provide on-site management of learning center programs and activities to ensure: (1) adequate staff, (2) needed facility space, and (3) needed supplies are available.
  • Responsible for resolving and documenting issues/problems as identified by learning center staff, volunteers, parents, students and/or campus administrators.
  • Serve on campus Planning Panel with grant staff, campus and community representatives, as appropriate.
  • Work with the Mentor/Partnership Coordinator to identify, recruit, and sustain mentors and community partnerships from a broad range of potential mentors and partners, including but not limited to local businesses, LISD Council of PTAs, social/civic organizations, faith-based organizations, public and private agencies, and retired citizen organizations.
  • Document background checks for all mentors and volunteers.
  • Coordinate matches between mentors and students, and facilitate introduction of mentors to students.
  • Effectively communicate program needs, goals, and accomplishments to Grant Supervisor, and Mentor/Partnership Coordinator.
  • Coordinate Learning Center planning and implementation with Grant Supervisor and Instructional Specialists.
  • Utilize a cohesive, positive, research-based discipline management system for after-school and summer programs.
  • Coordinate and assist in curriculum development and deployment for student instructional and enrichment activities and parent involvement programs.
  • Collaborate with Communities in Schools personnel and Learning Center staff to develop, implement, and evaluate new Learning Center deployment and to build long range plans for growth/expanded services.
  • Utilize mentor relationships and community resources to ensure quality programming, improved student in-school performance (academic and behavioral), increased attendance, and increased parent involvement.
  • Collaborate with Grant Supervisor, Mentor/Partnership Coordinator, and Instructional Specialists to facilitate parent involvement activities, providing experiences that enable parents to partner in their child(ren)'s education and to attain skills requisite for employment as needed.

Case Management

  • In conjunction with Mentor/Partnership Coordinator, and CISNT Campus Manager, develop cohesive and effective programs, projects and activities that meet CISNT's six programming components.
  • Enroll, assess student needs, develop a case management plan, and provide ongoing case management support for students in the Learning Center.
  • Provide student services, including supportive guidance and counseling, academic and enrichment enhancements, mentoring and tutoring, and others as identified.
  • Track and assess student progress and document services in case log.
  • Collaborate with regular school day CISNT Campus Manager to coordinate identified needs and services for students and families, including health and human service linkages, pre-employment and employment training.
  • Coordinate matches between mentors and students, and facilitate introduction of mentors to students.
  • Collaboratively work with the school day CISNT Campus Manager as a liaison between the school and CISNT.
  • Work with socially and economically disadvantaged youth and their families, including home visits and other forms of communication to establish rapport with students and their families throughout the school year.

Personnel Management

  • Orient mentors to Learning Center programming, connectivity to school-day, and goals. Demonstrate commitment to LISD, CISNT, and grant missions.
  • Effectively communicate program needs, goals, and accomplishments with Grant Supervisor, Instructional Specialist, and/or Mentor/Partnership Coordinator.
  • Proactively advocate for constructive relations between grant staff, mentors, community partners, and school and district staff.
  • Participate in quarterly Advisory Council meetings to apprise LISD, CISNT, and community partners of grant progress.

Professional Growth

  • Assume responsibility for personal growth to improve job performance.
  • Assist with planning and development of mentor training in collaboration with Grant Supervisor, Mentor/Partnership Coordinator, and/or campus-based Instructional Specialists. Provide an orientation specific to the Learning Center in order to acclimate the mentor to the campus and campus specific policies and procedures.
  • Conduct teacher training, as appropriate, to maximize mentor productivity and connections between Learning Center and the regular school day curriculum on all target campuses. Goal: Facilitate cohesive, quality instructional and enrichment programming for students and parents served by targeted campuses.

Program Evaluation

  • Systematically and continuously monitor campus-based Learning Center mentor, enrichment, and parent involvement offerings, coordinating for efficacy in meeting student and family needs. Make mid-course adjustments rapidly as appropriate for goal attainment.
  • Work with CISNT Campus Manager to verify all student-mentoring services have been documented in the student's case log.
  • In conjunction with campus-based Instructional Specialist, collect and analyze data weekly/monthly for quality assurance. Submit weekly/monthly reports detailing student data (attendance and discipline referrals in after-school program/summer program), family participation/services, mentoring activities, community and staff involvement, and proximity to grant goal attainment to Grant Supervisor and Mentor./Partnership Coordinator.
  • Systematically and continuously monitor mentor/mentee activities, enrichment programs on each campus, and parent involvement coordinating for cohesiveness and quality implementation.
  • Other duties as assigned

Supervisory Responsibilities:

  • Supervise learning center enrichment support staff, and volunteer mentors.

Working Conditions:

  • Maintain emotional control under stress.
  • Frequent, prolonged, irregular hours.
